We have created a facebook page and other social media to help people be informed on what we do in the academy. Please grant permission to post photos/videos of group shots participating in Futsal. Please go on the Facebook page and like @dtbfutsal and keep up to date with all of what we have to offer in our academy. Thanks so much for your continued support.

I give permission for my child, the participate of the academy, to be photographed during Futsal related activities. 

I understand and agree to give DtB Futsal Academy permission to post photos or videos on Facebook for marketing purposes.

This information will be immediately posted online in order to promote the academy. Data will be deleted immediately from devices and stored immediately on our social channels.
